Outreach Support Worker
We seek warm, professional support workers to work for our Manchester LD Outreach Pilot service.
Successful candidates will provide flexible, personalised support to people with learning disability and autism support needs to enable them to develop the skills to lead independent lives, promote their quality of life and personal development.
You will engage with service users who receive support from our Manchester Learning Disabilities outreach service, building trusting therapeutic relationships, working to encourage a positive life outlook and coping skills. Your role as outreach support worker will include visiting service users in their own homes or in other settings as part of a structured approach. You will work closely with other professionals and agencies to provide a co-ordinated service which meets the identified needs of the individuals. A full, clean driving licence and access to a car is essential for this role.
Our service operates 7 days a week, 365 days a year and you will be expected to work on a rota system undertaking shift work as required. Evening and weekend work is expected.
