Creative Support have recently secured an exciting new contract to provide an Independent Living Supported Housing Service to vulnerable adults and older people in Leicester.

 

The Independent Living Supported Housing Service is a highly responsive housing related support service for people with a range of needs including learning disabilities, mental health support needs, physical disabilities, sensory disabilities, those being discharged from hospital, or have needs relating to a long term or enduring health condition. This support is provided to service users within supported tenancies.

 

As well as providing direct support to service users, the Community Housing Team Leader will:

 

  • Lead and provide day to day management for the support staff team.
  • Work closely with partner landlords, other professionals and agencies
  • Provide a coordinated personalised service which meets the identified needs of individuals, promotes their recovery, reablement and maintains their quality of life
  • Ensure the quality provision of the service
  • Maintain all records required to demonstrate quality
  • Provide staff supervision and management
  • Undertaking initial needs and risk assessments for new clients
  • Ensure positive move on into independent accommodation.

 

As each service user receives a few hours of support per week, the aim of the service is to provide enabling housing relating support  to achieve positive move on, reduce the risk of losing accommodation, provide help and support in acquiring the skills to maintain and retain independence, help to maintain positive health and wellbeing to reduce or avoid crisis in health and wellbeing, improve social inclusion, enhance independence and contribute to improved and sustained quality of life for vulnerable adults.
