Manchester Mental Health Referrals for Supported Accommodation

Creative Support is a not for profit provider of personalised services – supporting people to enjoy their lives and reach their full potential. The aim of our Manchester Mental Health Referral Service for supported accommodation is to provide high quality, personalised housing related support for individuals with a range of mental health needs. Together with our tenants, we work in partnership with other professionals and health care teams to identify all needs, setting realistic and achievable goals for those that we support.

We work in accordance with the recovery approach and our services have a move-on pathway to reflect this; from 24 hour services through to supporting people in their own tenancies. When we receive a new referral, we ensure that each individual is aware of the expectations of personal development and outcomes. Where appropriate, we will also implement a move-on plan within the first 6 months of an individual commencing with the service.

The Referral Process  

To make a referral you will need to complete the Referral Form and email it back to

Please include the completed Declaration to Consent Form and a copy of an up to date MANCAS – if this is not available the last MANCAS with an up to date Support Plan and current Risk Assessment/Management Plan will be acceptable. (Please note we will be unable to start the referral process without this information.)

If you wish your referral to be considered for more than one service you will only have to make one referral via the single point of access. The dedicated Referral Co-ordinator will log the referral and discuss with the appropriate Creative Support area manager.

The only service that will still take direct referrals is The Shawe (Lone Parents Service). Referrals for this service should be made direct to the scheme manager
