Short Job Description:

Creative Support is a provider of person centred support and care to people with learning disabilities, mental health needs, autism spectrum conditions, and older people.

We are currently looking for a Support Worker to provide person centred support and care to a individuals with learning disabilities and complex needs in their own tenancy within the Eccles area. Support is to be provided on a 1:1 or 2:1 basis when participating in community based activities.

You will be an enthusiastic individual who is committed to providing person centred support to people with learning disabilities. This role will include:
• Providing personal care and practical and emotional support
• Support the man to participate in community and social activities
• Collaborating with the service user, his multi-disciplinary team and co-workers to provide a consistent service and to maximise service user outcomes

Understanding of complex communication approaches and behavioural management is desirable.

You will be expected to work flexibly to meet the needs of the service; as this service operates over 24 hours a day, this will include weekend and evening shifts on a rotational basis.

After successful completion of the probationary period, you will automatically be enrolled in the NVQ level 3 Health and Social Care Diploma and you will receive on-going training to best meet the needs of the service users.
