Short Job Description:

Creative Support is a high quality provider of support to individuals with a wide range of care needs including learning disabilities, mental health needs, autism spectrum conditions and older people. Employing over 5,000 staff members, we are continuing to grow at a fast pace nationally and are looking for dedicated, caring individuals to join our staff team in the Meir area.

We are looking for enthusiastic individuals to provide person centred care and support to older people. Your role will include:
• Providing personal care and practical and emotional support in service user homes
• Supporting service users to maintain appointments and to life a healthy lifestyle
• Identifying and accessing opportunities for social inclusion and maintaining a valued life
• Maximising service user outcomes through joint-working with the service user, their family and friends, and other involved professionals and agencies

You are expected to be flexible in hours, as this position will include evening and weekend work to meet the needs of the service.
