Internal Training Application Form: Classroom & E-Learning

Requests to attend training should be made via your workplace supervisor or manager. An Internal Training Application Form should be completed and signed by your manager. We apologise but we cannot take training requests over the phone, as we need to know that the manager agrees to the training and that the employee is aware of the date requested. This is also why we need to have the delegate’s and the manager’s signature on the form – so we know the person can be released from rota on the date requested.

This will then have to be authorised by your Service Manager. Any unsigned and non-handwritten signed requests will be declined. Please forward your request to the Training Department either by email training.applications@creativesupport.co.uk or fax 0161 228 0560.

Delegate lists for the training are compiled on a first come first served basis.

If you are unable to attend training, please let us know 2 weeks before the course is due. Otherwise, your service will be charged.
